TELL Partnership Order Form

Instructions

1. Fill out and submit the form below
2. Creative Works will email you an invoice
3. Pay online or send a check
4. Instruct students without accounts to set them up on tell.byu.edu with the email address you have submitted for them
5. After payment is received, we will add the TELL materials to these accounts

Please note: Materials orders can be made for only one class at a time (or we will be unable to match materials to accounts). Every student listed should get all the same requested materials. If you think you need an exception, please contact catalog@byu.edu.
